Locate the activity name, date, location, supervising contact, transport arrangements and any stated cost. Compare those details with the notice that accompanied the form. If a page refers to an attachment you did not receive, ask for it before deciding.
Make questions specific: where is collection, what equipment is needed, and who should be contacted if plans change? A general AI explanation cannot confirm the school’s current arrangements or whether an activity suits a particular child.
Read each permission separately
Look for separate choices about participation, photographs, information sharing or transport. Do not assume that one signature answers every checkbox. Ask the school how optional items and required items are distinguished if the form does not say.
Notice what personal information is requested and why. The Privacy Commissioner’s guidance encourages people to ask how their information will be used. Avoid posting a completed child’s form in a public forum or including unnecessary identifying details in a request for help.
Check the return process and keep a copy

Before sending, check that the answers are complete and accurate. Do not use AI to invent medical information, emergency contacts or a guardian’s consent. Keep a private copy and, where available, a submission receipt so you can refer back to what was provided.

An invented excursion form includes one box for participation and another for photographs. A reader who is unsure whether the photograph choice is optional should ask the school, rather than assuming that agreeing to the trip automatically answers both choices.
